Sample form for evaluating the head of an organization
The evaluation of the executive director/CEO is typically the responsibility of the board of directors. It is the board’s duty to determine if the ED is meeting their individual goals, as well as the goals of the organization.
If you’re looking for inspiration on how other organizations are facilitating this process, you’re in luck. Here’s a sample evaluation form shared by Joe Lisella, ED at Animal Protective Foundation. “Each board member completes it,” explains Lisella, “and then the president calculates the final score and shares comments during the annual review.”

Very disappointed that there is no mention of volunteers, who are often the backbone of a non-profit organization — no mention of expressing appreciation, recruiting, training and retaining them. Also no mention of building and reinforcing an open, transparent workplace culture, where there is no bullying, intimidation, or dismissiveness of newcomers and new ideas. And there was no mention about recruiting and retaining quality employees, and avoiding unnecessary turnover, which can rob an organization of institutional memory and efficiency.
